Personal Data being processed relates to the information taken in order to ensure that the limitations of confidentiality and ethical reasons to refer in relation to harm to self or others, child protection, and the safe guarding of vulnerable adults and children. This also includes details in relation to national security.
In addition, we hold client notes in order to maintain fluidity to the sessions for the benefit of the client and the therapist.
How and when is Data collected?
Green Counselling will conduct an initial assessment at the beginning of the therapy session, and will regularly conduct reviews to monitor progress. It is at these times that personal data may be collected and or amended, and with the client’s verbal consent to do so.
Where is the data stored?
The data is stored in hard copy, hand written notes, and are kept in a locked and fire proof cabinet. Access to these notes is restricted to Michelle Green. The offices are protected by a security system.
Who has Access?
Michelle Green is the only person to have access to these files.
How long is the Data stored?
Our records are to be kept for a maximum of five years, after this time records are shredded.
Who else is the data transferred to?
As required by the statute and General Data Protection Regulation only.
- Third party, Power of Attorney
- Referral to GP, Social Services or Police in case of risk of harm to self or others, Child Protection or in the interests of National Security
- Court Order
- Social Services, in case of work undertaken on their behalf
- In any other event, should data be transmitted to third parties, for example school SENCO, we would ask for your authority.
To be able to provide you with the therapeutic services set out in our website (www.greencounselling.co.uk). Green Counselling must hold and process personal data. We use this information to hold on record in case of the need to refer to any other agency as highlighted above. Client notes may be taken to keep the fluidity of the session for the benefit of you the client and the therapist. You have the right to request to see your notes at any time.
Please take the time to read our policy statement. We ask that you are happy to continue with Green Counselling, and that you are happy with our data protection policy. This may be given by way of verbal acknowledgement and consent and by the voluntary signature from you the client. A copy of our data protection policy can be requested at any time from us.
